It is an official document provided by the Louisiana State Board of Private Investigator Examiners, containing his license number and address. The license can be found in his inventory since the very beginning along with the Lasting Debt.

After Carnby is pointed to the empty room by The Dark Man, he finds himself in a state of transe, believing he is a patient in Derceto. He finds a tear in the wall that points him to his repressed memories and the combination of the safe, hidden within the license number.
